Medical Insurance and Payment Info

Payment is expected in advance or at the time of service and is ultimately the patient’s responsibility. Your insurance plan may dictate your payment arrangements. Payment is accepted in the form of cash, check, CareCredit, credit card, HSA card, Flex spending card, and auto MedPay. Customized payment plans are available to help you meet your health care needs.

Our office may give you a superbill statement to submit to your insurance. Your individual policy determines what benefits your insurance will cover. You may want to verify your insurance benefits.

The lowest fees are typically DDP. First visits average $105-155. Subsequent visits $55-105. Solo dry needling area treatment $70. Vasopneumatic cupping therapy is $70. No billing is allowed with DDP.

Other insurance companies and discount plans may cover care in our clinic, please contact your carrier or email your card to info@BroomfieldWellness.com along with your DOB for verification. If you don’t see our clinic or your provider’s name on your insurance carrier’s web site it does not necessarily mean that you do not have coverage in our clinic, it could mean that your insurance plan uses an intermediary network. We are not contracted with intermediary networks but can accept out-of-network benefits. We do not accept intermediary networks due to their extremely low reimbursement rate.
